Case Manager - Men's Residential

Residential Case Managers are responsible for non-clinical direct
and indirect substance abuse treatment services to behavioral
health clients seeking or obtaining services from the agency.Case
Managers are usually assigned a caseload that consists of 60%
direct service time and 40% indirect service time. Direct service
includes assessment interviews, individual, family and group
education and/or resource support and discharge planning. Indirect
service time includes supervision, staff meetings, telephone calls,
treatment authorizations, and all paperwork associated with client
EXAMPLES OF RESPONSIBILITIES:1. Develops positive working
relationships with supervisor, co-workers, and members of the
clinical team.2. Develops appropriate professional relationships
with clients within the framework of the agency and within each
individual's scope of practice.3. Abides by the Code of Ethics.4.
Conduct and write assessments within the contractual, funding, and
licensing timeliness specified in the Program.5. Collaborate with
clinical staff in the development and implementation of treatment
plans, treatment plan reviews, and discharge/continuing care plans
within the contractual, funding, and licensing timelines.6.
Facilitate psycho-educational groups, provide individual support
where needed.7. Include members of the client's support network in
the treatment process.8. Develop effective networking relationships
with outside contractors, referral sources, and community
agencies.9. Supervise urine samples for testing, and perform
breathalyzer testing.10. Conduct room checks/searches in
residential programs.11. Supervise self-administration of
medication with clients.12. Assist with oversight and supervision
of daily tasks of residential programming with clients; i.e.:
chores, meal prep, smoke breaks.
Location: 27 N. Elm Street, Waterbury
Full-Time position - 40 hours/week.Payrate: $15.50/hr.
Education and Experience:
- Bachelor's degree or Associates's Degree with two years of
experience in the field of substance use and dual diagnosis.
Certified Alcohol and Drug Counselor (CADC) preferred.
- The candidate must be flexible with a high degree of
motivation, exercise good judgment, and be able to work
- Demonstrated experience in a similar program, internship, or
related employment or specialized training required.
- Solid computer skills are required, Office 365 is helpful.
- Must have a valid Drivers license, clean driving record, and
- CPR/First Aid certification must be obtained and maintained.
Wellmore will pay for the classes onsite.
- Must be able to walk multiple flights of stairs. (no elevator
in the group home)
