Job Description:

SCOPE OF POSITION:# Identify, collect and maintain timely, accurate and complete information about cancer patients diagnosed and/or treated within the organization.# Disseminate information in accordance with federal and state laws and regulations, facility policy and professional ethics. # RESPONSIBILITIES: Identify malignancies by referencing primary and secondary sources of information. Abstract patient demographics, diagnostics, history and extent of disease, cancer directed procedures and treatment, cancer stage, and related information from available medical records. Obtain lifetime follow-up on cancer information such as disease status and vital status. Preserve the privacy and security of patient health information, adhering to federal and state laws and regulations, facility policy and the National Cancer Registrar Associations Code of Ethics. Support Cancer committee by preparing the agenda and minutes. Coordinate monthly multidisciplinary cancer conferences Attend cancer conferences to monitor compliance with ACoS/CoC standards Support annual physician quality review of cancer registry data. Complete Survey Application Record (SAR) annually Submit registry data annually to the National Cancer Data Base (NCDB Submit registry data monthly to the Rapid Quality Reporting System (RQRS) Submit registry data to the Connecticut Central Tumor Registry quarterly Prepare Survivorship Care Plan reports for physician delivery to qualified patients. Maintain the cancer registry policy and procedure manual. Assist clinicians and other researchers in cancer related studies and research. Prepares routine and special statistical reports, narratives and graphic representations.# Distributes as appropriate. Perform other related duties as required ################################### REQUIREMENTS: Graduate of cancer registry management or health information management program.# Preferred CTR or eligible. RHIA or RHIT preferred, with two years of cancer registry experience may be substituted for the CTR qualification or Associates degree with two years previous Cancer Registry experience. Proficient in ICD-0, Collaborative Stage, AJCC 8th edition, FORDS, MP/H and Hematopoietic manual. Proficient in Cancer Program Standards as published by the ACOS/CoC Ability to read and understand medical terminology and anatomy. Must have basic computer skills and ability to access internet and other programs applicable to Waterbury Hospital procedures. Perform automated functions that fall within job responsibility. Excellent verbal and written communication skills Ability to maintain confidentiality. #LI-JT1
