Summary Serves as an active part of a team that provides in-home assessment and clinical treatment services to parents and infants (0-3) who present with significant substance abuse and parent child attachment needs. The goals are to help parents' maintain sobriety, develop proper attachment to their infant and acquire the skills necessary to become effective parents so that they can ensure a safe and secure home environment for their children. This position requires collaboration with community providers and funders. Education Master's Degree in behavioral health (psychology, social work, counseling, marriage and family therapy, alcohol and drug counseling). Credentials and Licenses CT LCSW license preferred. Clinician II must have a Connecticut license as a LCSW, LMFT, LPC,Ph.D., Psy.D. LADC as appropriate for services. Certified in TCI, First Aid and CPR. Experience

  • Successful experience in the provision of substance abuse and mental health services with children, adolescents, adults and/or families, preferably a minimum of two years.
  • Bilingual preferred (English/ Spanish).
  • Beginner to intermediate experience with Microsoft Office products including Word and Outlook.
  • Experience in, or the ability to learn how to use an Electronic Medical Record (EMR) system which includes appointment scheduling, authorization tracking, assessments, treatment plans, progress notes and discharge summaries. Job Requirements:Main Duties & Responsibilities
    -Performs comprehensive intake assessment that results in diagnosis and recommendations for psychiatric and substance abuse treatment for adults and their children and assists with appropriate referrals and linkage to others services, if appropriate. -Develops treatment goals and objectives with adult and/or family. Follows Family Based Recovery Program Model and appropriate treatment modalities (individual, family, group, couples) that promote the amelioration of substance abuse and/or psychiatric symptoms and behaviors exhibited by clients. -Evaluates client risk on an ongoing basis and formulates and implements appropriate safety plan. -Collaborates and coordinates with external agencies such as schools, PCPs, pediatricians, DCF, probation, court and other social service agencies. -Communicates with other clinical staff, case manager,DCF, pediatricians, and other members of the provider team and completes documentation within program and CMHA time-frames. -Provides crisis intervention to family in the home, office, or community if need arises. This includes on-call 24/7 coverage of families on caseload. -Performs urine and breath toxicology screens. -Maintains accurate documentation and submits within program and funder timeframes. -Maintains appropriate caseload as determined by CMHA and funder. -Performs other related duties as assigned.
