POSITION TITLE: Bilingual Clinician Case ManagerPROGRAM:
Wellmore, Inc. (outpatient; Hispanic & community placements)GENERAL
STATEMENT OF RESPONSIBILITIES:The Clinician Case Manager is
responsible for direct services with children, adolescents and
their families including but not limited to evaluation/assessment;
home& community based services; outreach; case and care management;
individual and family therapies; crisis intervention; record
keeping; collaboration with other professionals and team members
and related community services (including consultation and/or
community and/or school placements). He/she may be assigned to
respond to all calls for assistance either by telephone or in
person (as clinically indicated).SUPERVISION: Supervised by: Team
Leader (or designee)Supervises: Interns and/or those designated by
supervisor.EXAMPLES OF RESPONSIBILITIES:To provide comprehensive
evaluations, treatment plans and treatment of children, adolescents
and families. This includes evaluation/assessment; home and
community based services; outreach; case and care management;
individual, group and family therapies, as well as crisis
intervention.To implement treatment plans that may include
psychotherapy and clinically required support services including
but not limited to translation, transportation (only as per agency
policy) advocacy and linkage to other needed services.To provide
community based outreach and treatment services as defined by the
treatment plan and/or client need, in a variety of settings
including but not limited to: agency offices sites, client homes,
schools, emergency rooms, shelters, police stations, etc.Maintain
confidential and/or any other required program documents.Maintain a
level of productivity as determined by the CEO and assigned by the
program supervisor.To work collaboratively as a member of a team
with other professionals and concerned parties involved in cases.To
assist in the organization, marketing, planning and implementation
of the program.Actively participate in trainings and staff
meetings.To serve on agency and/or community committees.Actively
participate in supervision and report directly to the supervisor.To
supervise assigned staff and/or student interns.Other such duties
as assigned by the Supervisor.QUALIFICATIONS:Must have the ability
to maintain the utmost confidentiality of both agency and client
matters. Must understand and be able to display the clinical
knowledge andexpertise necessary to perform crisis intervention.
Must be able to display knowledge and expertise in clinical theory
and practice as it relates to the populations served by Wellmore,
Inc. Must be capable of working as a part of a team and be able to
effectively communicate with both families and professionals (in
writing and verbally). Working knowledge of the greater Waterbury
area is a must! Ability to read, write, translate and interpret
Spanish.EDUCATION & EXPERIENCE:A Masters Degree in marriage and
family therapy, social work or related human services field with at
least a one year clinical internship working with children and/or
adolescents or families is required. A Connecticut license as a
Marriage and Family Therapist, Professional Counselor, Licensed
Clinical Social Worker or Doctorate Psychologist preferred. MSW
candidates must possess a LMSW at a minimum. Two years post
graduate clinical experience working with children, adolescents or
families is preferred. Experience in family therapies as well as
family systems theory and practice is preferred.by Jobble
